Mail-In Sperm Banking Fees (effective January 1, 2009)

UrgentPak Mail-In Sperm Banking Kit (per specimen)     $125/each

 UrgentPak it is the only transport kit that has been developed to provide an optimum method for maintaining the proper environment of your specimen during transport.  It complies with both National and International regulations (IATA, ICAO and 49CFR) for shipping of Human Biologic Specimens. The fee for the UrgentPak kit includes roundtrip shipping by FedEx. Please note that UrgentPak fees are non-refundable.

Specimen Processing and first year storage            $500 first Specimen
                                                                                  $250 each additional specimen

Additional years storage                                           $350 per year

Payment Policy:
Fees for the UrgentPak are paid at the time of request. Fees for specimen processing and specimen storage fees are paid when the specimen is received at New York Cryo. Any fees for additional tests requested such as semen analysis are due when requested by the depositor. Additional years of storage are due 30 days prior to the anniversary date of storage. Quarantine fees are charged monthly on the anniversary date of storage. These charges are payable by credit card (VISA, Mastercard, American Express or Discover). Credit card must be current and either paid in full initially or authorized for automatic withdrawal . To keep our fees as low as they are, the depositor must agree to store for a minimum of 3 years.
